commission: a degraded queue read was treated as empty, risking double-filed tickets [fixed]

Module: runtime/commission.py. fetch_tickets returns ([], reason) on ANY failure, indistinguishable from an empty system; the reconcile consumed it without checking, so a lost ledger plus a degraded read on --commit would file the whole graph blind -- duplicate governed tickets that cannot be cleanly deleted. Fix: fail closed -- refuse to file when the queue read returns no tickets (congruency is never empty). Status: fixed in the commissioning commit.
